MDLIVE Health Coaching

It's easier to manage chronic conditions when you have help. The MDLIVE Health Coaching app supports you in achieving your health goals and is available exclusively to patients prescribed health coaching by their MDLIVE primary care doctor.

The app gives you:
•	Personalized health insights and advice. Nutrition, fitness, and self-care recommendations based on your condition, goals, care plan, and data from Apple Health®. Stay on top of your progress with insights on trends and patterns.

•	The ability to easily record and track your vitals. Enter manually or connect your health monitoring device to the app and get alerts and recommendations. 

•	Medication and activity reminders. Stay on track with your goals with daily notifications to take your medications or get moving.

•	An easy way to share your progress with your MDLIVE doctor. Celebrate your wins and share your challenges with your doctor and MDLIVE care team directly through the app.

Use the app if you:
1.	Were prescribed health coaching by your MDLIVE doctor
2.	Need reminders to stick to your plan or take medications
3.	Would like personalized support to help you through your journey
4.	Want to learn how to have a healthy lifestyle
5.	Want to improve your overall health and manage your condition better with your care team

The MDLIVE Health Coaching app works alongside the MDLIVE patient portal.  You'll continue to access your MDLIVE patient portal while using the app to schedule follow-up care with your MDLIVE doctor, receive appointment reminders and access your health records.

The MDLIVE patient portal and MDLIVE Health Coaching apps are secure and compliant with federal and state laws, including the Health Insurance Portability and Accountability Act (HIPAA).
