Our team of Duke University doctors and scientists are investigating whether the health data from your smartphone and smartwatch can help determine whether or not you have a COVID-19 infection, and how severe the infection is expected to get. We will ask for your permission to access the Health app data on your phone and use this data along with daily surveys to examine whether you have Covid-19 symptoms. Our daily survey asks two simple questions each day. We will explore whether there is a relationship with your smartphone and smartwatch data.
Worried about survey accuracy
Often after an update the app will ask you to complete the survey twice and I haven’t found a way in app to report the error.

Bugs Seem to Be Resolved
I don’t normally write reviews, but this got such low rating due to bugs I felt I should comment that at least for me they seem to have been addressed by the time of this review (Oct ‘20). I’m on an iPhone 11 and the notifications and surveys come through fine. No crashing. It’s important research so don’t let the older versions of the app dissuade you.
Good idea but the app can be unstable
On my iPhone 8, the app has basic functionality but as others have noted the screens are poorly designed and text is cut off or overlaps where it should not. The bigger issue for me is that the app completely crashes my phone if I try to do anything other than answer the surveys. During initial setup, the app crashed and completely locked up my phone. I had to do a hard reset to get the phone to function again. Today, I tapped on the where can I get tested icon to see whether my local public testing site was listed but that caused the app to shutdown and then my phone locked up again with the screen going nuts and then black. Another hard reset and I’m back to normal. So, the app needs some coding fixes but this is a very worthwhile project so I will stick around and just answer the surveys as long as the problem doesn’t get worse. If this was a commercial app, I would probably give it two stars and delete it.
Doesn’t Scroll Properly
When I start the survey in the app it doesn’t scroll properly. For first question, “who were you near?” Only the “other” option shows with the text box, and even then only partially, but enough to enter an answer. I tried scrolling but it won’t scroll so the rest of the screen is visible. This is going to skew your results big time, and drive people away in frustration. I’m happy to participate but not if I’m going to spend hours troubleshooting a glitchy app. iOS 13.5.1.
Not Stable
I’ve been taking the email surveys for the past month and was happy to give the app a try. Disappointed that all personal info has to be entered again even for existing participants. The app crashed 3 times just trying to get through that questionnaire. Finally managed one daily survey and it crashed again. Needs some work. I can see participation dropping due to frustration with this app and/or its ability to function long enough for the surveys to occur.
App crashes
I installed the app and went to take my first survey. The app crashes while entering data to set up the survey (email address, gender, etc. ) I’ve tried multiple times and emailed the study, but no success and no help or response from the study. I will just delete the app and go back to email surveys.
