Upload documents to the NY State of Health (NYSOH) health plan marketplace
When you apply for health insurance through the NY State of Health (NYSOH) marketplace, you may need to provide supporting documents to complete your application. With the NYSOH Mobile App, you can take a picture of your documents, or choose a photo of your documents from your device’s album, and upload it to your account. The NYSOH Mobile App now also allows you to view your eligibility and enrollment dates, program information, and more.
The NYSOH Mobile App is quick and easy to use. Following a simple log in from your phone or tablet, you can choose to upload a document to your account or navigate to your account overview to view your account information.

please be aware...if you have an apostrophe in your name it will not recognize it. this is a government app and it is not recognizing my government issued name. this needs to be addressed and rectified.

no information and no help. my insurance was canceled and I tried calling to recertify the agent told me this was the only way I could do that and refused to help me. this app is asking for some AC number I don't have so I can't use it to log in. just great how every system in this country is broken.
