Care4Today® 2.0 Pro

Care4Today is a digital care coordination communication tool that allows healthcare professionals to connect, educate and support their patients through an entire care pathway.

Providers will be able to use Care4Today 2.0 Pro to:
- Track patients' progress through their treatment pathway

- Provide high quality learning materials at the right time in your patients' care;
- Provide forms, information and FAQs in a single digital platform;
- Communicate with patients and your care teams

- Get access to data and insights that allow for continuous improvement.

- Care4Today does not qualify as a medical device per applicable regulations, including the EU Medical Device Regulation No 2017/745.
- Care4Today does not perform an action on data different from storage, archival, communication or simple search.
- Care4Today is not intended for patient diagnosis or treatment.  Healthcare professionals only are responsible for taking decisions related to patient diagnosis or treatment.
- If patients have any questions related to their state of health at any point in their treatment, they should contact their healthcare provider directly.
